healthcare 2One of our goals in partnering with Health Care Systems is to delay or reduce the onset of Type 2 Diabetes in community participants of the National Diabetes Prevention Program (NDPP.)

Partners in Healthy Living works with providers to encourage referrals, support breastfeeding, and cessation services for patients using tobacco products. We help clinicians to be successful in working with patients who are obese or at risk for obesity, including referrals to community resources.

We have members of the Community Leadership Team whose work and organizations directly impact populations over 60. The Seven County Senior Federation, University of Minnesota Extension and MN Council on Aging are represented and their input directly impacts the community.

Our team in Isanti County Public Health has developed a partnership with Act on Alzheimer’s in Cambridge to provide education in the community about dementia and the benefits of early detection and treatment.

With evidence-based, community-level, comprehensive strategies, Partners in Healthy Living creates opportunities for healthy choices where people live, learn, work and seek health care. Over time this leads to better health, lowers health care costs and improves quality of life in Minnesota.

Cambridge Medical Center

Cambridge Medical Center:

• The Cambridge Medical Center-Hospital (CMC) has integrated the National Diabetes Prevention Program into their evidence based community class offerings.

In 2017, Fairview began the implementation of SNAP Rx to decrease food insecurity with populations in need by connecting them with local resources.

FirstLight Health System

FirstLight Health System has been a partner working SHIP initiatives since 2016.

  • In 2016, FirstLight Health System installed bicycle racks and signs at their Pine City and Hinckley sites.
  • In 2017, it added a hydration station at its Hinckley site.
  • In 2017, a breastfeeding station was added to its Mora site.
